Thank you for a rational, common-sense, science-driven article. The PC left is destroying itself with irrational nonsense that causes very predictable reactions by conservatives. Are some of these conservative bigots? No doubt. But to pretend that sex is "assigned" is idiotic and most people don't respond well to idiocy. I'm old enough to remember Cristine Jorgenson and of course the much later Renee Richards. They did their thing and most people didn't really care one way or the other (except, in the case of Richards, women professional tennis players. Luckily for them, Richards wasn't that good). Today's trans people are a different story. There's also evidence that trans self-identification is in part due to social contagion (as was the case with many eating disorder outbreaks).

I struggle a bit to wrap my brain around the fact that people w complete androgen insensitivity are in fact male, but do not have the same struggle classifying females with CAH as female. Upon reflection this may be due to the fact that CAH is almost always identified at birth due to the potentially life-threatening nature of the condition. However, as you said, CAIS may be missed at birth and only discovered during puberty due to primary amenorrhea. Thus the child and family (and pediatrician) would have believed all along that the child is female, which is an error. But objectively speaking, when an SDS condition is typically diagnosed should have no bearing on whether the person is male or female. I am now persuaded that people with CAIS are in fact male with feminine secondary sex characteristics. Thank you for helping me reflect and understand.

Colin, how would you classify cases of gonadal dysgenesis or true streak gonads? Would you defer to chromosome or internal genitalia?
